Texture LOD issue

For some reason, when I specify the min and max LODs and try to display the texture (just the texture on a single quad), 90% or so of it becomes blacked out after flashing briefly on the screen. If I don’t specify the LODs everything is fine. I’m certain all mipmap levels are present. This is on a Radeon 9700 pro. Any insight?

I have been using this, but I haven’t seen any problems with it. Do you have a sample?
