I'm having no joy with this. I get a "white square" like when I forget to enable GL_TEXTURE_2D.

Does anyone know how to correctly specify an 8-bit paletted texture using this extension?

To reply to myself:

remember to bind the texture first, then call glColorTableEXT, then call glTexImage2D... and it works

duh.
